What is MIH?

Mobile Integrated Health (MIH) is a program designed to provide care beyond emergency response. Instead of only responding to 911 calls, MIH focuses on helping patients stay healthy and supported in their everyday lives.


Our team works directly with patients in their homes and communities to provide follow-up care, prevent avoidable health issues, and help individuals navigate the healthcare system more effectively.


By focusing on prevention, education, and coordination, MIH helps ensure patients receive the right care at the right time.

Our Mobile Integrated Health program offers a range of services designed to support patients in their homes and communities:


  • In-home follow-up visits to monitor recovery and ongoing health needs
  • Post-hospital discharge visits to reduce readmissions and support recovery
  • Chronic disease support for conditions such as diabetes, heart disease, and more
  • Mental health and substance use follow-up with compassionate, coordinated care
  • Telehealth support and care coordination to connect patients with providers and services

Mobile Integrated Health services are available to individuals who may benefit from additional support managing their health outside of emergency care.

Mobile Integrated Health is designed to support and strengthen existing healthcare systems within Putnam County.


Our program works alongside primary care providers, hospitals, and community organizations to improve continuity of care. MIH does not replace primary care, but instead enhances access and coordination for patients who need additional support.
